This 50-minute symposium, "RCTs Galore: The Evidence We Have, The Answers We Don’t," is tailored for colorectal surgeons, focusing on recent randomized controlled trials (RCTs) that have shaped clinical practice while also highlighting persistent knowledge gaps. The session will begin with a discussion on the ANCHOR trial, exploring its success in anal cancer prevention and the challenges of extrapolating high-resolution anoscopy to the broader at-risk population. Next, the symposium will examine the widespread adoption of indocyanine green (ICG) fluorescent angiography for assessing anastomotic perfusion, reviewing the current trial evidence, and discussing whether its use is justified despite the lack of definitive outcome data. Finally, the session will address the evolving role of neoadjuvant chemotherapy for colon cancer, summarizing the benefits and limitations of this approach and discussing the critical question of when and for whom it should be considered. By the end of this symposium, attendees will be able to describe challenges in applying anal cancer prevention strategies, review the current evidence on ICG, and summarize the benefits and limitations of neoadjuvant chemotherapy for colon cancer.
