Purpose/Background: The rates of early onset colon cancer continue to rise in the United States. More recent studies show, in early onset colorectal cancer, right-sided tumors were associated with lower 5-year overall survival compared to left-sided tumors. At time of diagnosis, right sided colon cancers are more likely to have poorly differentiated and advanced tumors compared to left colon cancer. Our study aims to identify differences in overall survival based on location of the primary lesion.

Methods/Interventions: A retrospective review of the National Cancer Database (NCDB) was performed from 2014-2022. Patients diagnosed with adenocarcinoma of the colon and rectum were included in this study. Patients over the age of 50 years and those with unspecified location of the primary tumor were excluded from this study. In this study, we identify the right-sided colon cancers as tumors located in the cecum, ascending colon, hepatic flexure, transverse colon, and splenic flexure. Left-sided colorectal cancer included descending, sigmoid, rectosigmoid, and rectum.

Results/Outcomes: The average age of diagnosis was not significantly different amongst the individual groups. A disproportionately large number of men compared to women were diagnosed with colon cancer before the age of 50. Nonetheless, the distribution of male to female diagnosed with colon cancer was not significantly different between each site of colon cancer. When comparing the various primary sites of the right-sided colon cancer, there was a significantly higher mortality rate in cecum compared to the other regions (ascending, hepatic flexure, transverse and splenic flexure).

Conclusion/Discussion: Right colon cancer has worse 5 year overall survival compared to left-sided colon cancer amongst adults under 50 years of age. Further analysis shows cecal tumors have the highest 5 year mortality rate compared to other primary sites of colon cancer. Additional research is warranted to investigate underlying causes of the disparity in overall survival between left-sided and right-sided colon cancer.
