Purpose/Background: A significant proportion of patients with Inflammatory Bowel Disease (IBD) require surgery through the course of their lives. Current guidelines recommend the use of shared decision making in planning intervention pathways. One way of introducing this in practice and improving patient education is the routine use of Patient Decision Aids (PDAs). This study aimed to assess the use of PDAs in IBD and their impact on surgical decision making.

Methods/Interventions: Five databases (MEDLINE, PubMed, Embase, CINAHL, CENTRAL) were searched from November 2002 to June 2024, for studies assessing the utility of patient decision aids in medical and surgical decision making in adults with IBD. Two independent reviewers screened records, performed data extraction and risk of bias (CASP) and assessed decision aid quality using the International Patient Decision Aid Standards checklist. Results are summarised narratively.

Results/Outcomes: Five studies were included from a total of 1945 citations. These included 424 patients and 21 clinicians. The domains assessed were impact on decision making, knowledge, satisfaction, treatment preference and/ or changes, usability, and acceptability. Three out of four included studies found that PDAs improved overall certainty regarding treatment choices with fewer patients opting for 'no therapy' (p < 0.05). Only one study assessed knowledge in the context of surgical decision making and found a significant improvement using a 13 item questionnaire (9 to 12.5; p = 0.06). PDAs had high satisfaction scores amongst both patients (92%) and clinicians (93%). Two out of three studies that assessed impact on treatment preferences found that patients who had used a PDA were more likely to opt for combination therapy overall compared to standard care. Additionally in one study 6 participants changed their surgical preference after the PDA was administered. Both patients (82-97%) an clinicians (65-96%) found the PDA acceptable to use.

Conclusion/Discussion: This review supports the use of PDAs in surgical decision making in IBD. Overall, we found a positive impact on patient knowledge, decision conflict, and patient satisfaction. Additionally, the majority of patients and clinicians found their usability to be acceptable. Further studies are needed to assess their long term impact and utility in a more heterogeneous population.
