Purpose/Background: Locally advanced rectal cancers requiring multivisceral resection are difficult to manage with safe CRM. minimally invasive techniques for such multivisceral resection should be adopted for good surgical ouctomes. sacrectomy are generally performed via open techniques in prone positioning. we present a case of Laparoscopic total pelvic exenteration with transabdominal distal sacrectomy with perineal first approach .

Methods/Interventions: our patient was a young male with PDAC low rectal cancer with poor response to treatment. response MRI had anterior prostatic apex abuttment with posterior S3-S4 sacrum involved with mucin changes

Results/Outcomes: we could successfully perform Laparoscopic total pelvic exenteration with transabdominal distal sacrectomy with perineal first approach in lithotomy position with minimal morbidity . post op course was uneventful.HPR was ypT0n0 with mucinous changes till prostate and presacral fascia.

Conclusion/Discussion: Use of Minimally invasive techniques with trans abdominal sacrectomy is a feasible option for pelvic exenteration with safe margins and should be advocated whenever feasible
