Purpose/Background: Single-port robotic transanal endoscopic microsurgery (SP rTEM) enhances precision, visualization, and ergonomics during local excision. The introduction of a SP r system enables improved access transanally, minimizing the technical limitations of TEM/TAMIS. We present a case demonstrating the capabilities of the SP rTEM for a post-neoadjuvant ymrT2N0 rectal cancer.

Methods/Interventions: A medically compromised 85-year-old patient initially presented with T3N1M0 low-rectal adenocarcinoma with downstaging to ymrT2N0, underwent neoadjuvant chemoradiation followed by excision using the SP r platform. Full-thickness local excision was performed, demonstrating the capabilities of the SP robot performing endoluminal surgery. The rectal wall defect was closed with a barbed running suture after washout with tumoricidal fluid.

Results/Outcomes: The patient was discharged on postoperative day 1 without complications. All margins were negative. At 12-month follow-up, the patient remains disease-free.

Conclusion/Discussion: SP rTEM represents a safe and effective minimally invasive approach for selected rectal cancer patients following neoadjuvant therapy. It offers enhanced precision, minimal invasiveness, and rapid recovery while maintaining oncologic adequacy and sphincter preservation.
