Purpose/Background: Minimally invasive extended resections are difficult to perform. This video demonstrates the feasibility of use of robotic technique to perform complicated pelvic exenteration in a locally advanced rectal cancer with sealed perforation.

Methods/Interventions: his video demonstrates our approach and technique to perform robotic total pelvic exenteration in a locally advanced rectal cancer with sealed perforation abutting the obturator internus muscle. A 35 years old gentleman with locally advanced carcinoma rectum with MR images suggestive of sealed perforation, who had received neoadjuvant chemoradiation followed by 4 cycles of FOLFIRINOX and was operated for robotic total pelvic exenteration with excision of obturator internus muscle cuff.

Results/Outcomes: Surgical Procedure was uneventful with operative time of 480 minutes and blood Loss of 200ml. Postopearative recovery was uneventful and the patient was discharged on post operative day 9.

Conclusion/Discussion: Extended resection surgeries are a feasible robotically under trained and experienced surgeons with the advantage of significantly reduced morbidity.
