Purpose/Background: To evaluate the feasibility and safety of laparoscopic "page-turning" five-step method for left-sided colorectal cancer with or suspected to have adjacent aortic lymph node metastasis.

Methods/Interventions: A total of 19 cases were retrospectively included. The dissection was performed layer by layer from the anatomical boundary, starting from the bifurcation of the abdominal aorta towards the left renal vein, with sharp separation to remove the anterior, pre-vascular, and lateral lymphatic tissues.

Results/Outcomes: All surgeries were completed laparoscopically without conversion to open surgery. The time for pelvic aortic lymph node dissection (PALND) was 83.6±7.8 minutes, with an estimated blood loss of 78.4±59.9 mL. There were 5 complications (all grade II), with no 30-day mortality. A total of 22.5±10.5 lymph nodes were harvested, with 4.6±6.3 positive lymph nodes, predominantly from the lateral stations.

Conclusion/Discussion: This method achieves standardized and complete lymph node dissection under clearly defined planes, with acceptable short-term outcomes and repeatability.
