Purpose/Background: Laparoscopic right hemicolectomy can be performed using either extracorporeal or intracorporeal anastomosis. The choice of technique may influence short-term postoperative outcomes, including anastomotic integrity, hospital stay, and oncologic specimen quality.

Methods/Interventions: A retrospective study was conducted including 149 patients with right-sided colon cancer who underwent laparoscopic right hemicolectomy with intracorporeal or extracorporeal anastomosis at four institutions in Bogotá (Hospital San José Centro, Clínica Los Nogales, Clínica Oncológica Colsubsidio 127, and Clínica de la Mujer). Variables analyzed included operative time, blood loss, anastomotic leak rate, postoperative ileus, hospital stay, and lymph node yield on pathology.

Results/Outcomes: A total of 108 patients underwent intracorporeal anastomosis and 41 underwent extracorporeal anastomosis. The intracorporeal group showed a longer operative time but significantly reduced intraoperative blood loss, a lower anastomotic leak rate, and fewer postoperative ileus events. Median hospital stay was shorter in the intracorporeal group. Additionally, lymph node harvest was higher in patients who underwent intracorporeal anastomosis, suggesting a more extensive oncologic resection.

Conclusion/Discussion: Intracorporeal anastomosis in laparoscopic right hemicolectomy is a safe and reproducible technique associated with improved short-term outcomes, including reduced morbidity and enhanced oncologic quality. This multicenter experience, developed within a specialized colorectal surgery training program, supports the routine use of intracorporeal anastomosis in expert colorectal units.
