This talk will explore the evolving role of non-operative strategies, including watch-and-wait and organ-preserving approaches, in managing rectal cancer among frail and elderly patients. Emphasis will be placed on balancing oncologic safety with quality of life, questioning whether total mesorectal excision remains the optimal standard in this vulnerable population.
