Purpose/Background: Locally recurrent rectal cancer after primary treatment completion most of them have systemic disease however some of them still have chance of cure with localised disease. Surgery in recurrent rectal cancer most of times done by open approach due to strong pelvic adhesions and distorted planes after primary surgery, however there is still chance of MIS approaches in such disease after careful patient selection and evaluation. We presents a case of recurrent rectal cancer post abdominoperineal resection after 2 years of Disease free interval with localised recurrence at the site of prostate and extending upto right Vesicoureteric junction. We have performed Robotic pelvic exenteration with bilateral vascular approach the right approach has been shown in video. After bladder and pelvic dissection the specimen extracted perineally which is closed with Bilateral gluteal flap and ileal conduit reconstruction done with mini-laparotomy by infraumblical incision.

Conclusion/Discussion: Application of robotic surgery in recurrent rectal cancer in current era help to maximise reach of safe resection.
