Purpose/Background: 36 year old female who developed severe pelvic sepsis. She required multiple extensive perineal and gluteal debridements for source control. She also underwent colostomy creation. She later developed a large posterior anal sphincter and gluteal defect. The defect this measured about 30% of the circumference of the sphincter.

Methods/Interventions: Combination repair with colorectal surgery and plastic surgery. She underwent overlapping sphincteroplasty and VY gluteal flap reconstruction. She also had reversal of the colostomy 3 months following the repair

Results/Outcomes: Patient recovered well. Good function with LARS score of 25; she mentions intermittent gas incontinence.

Conclusion/Discussion: Overlapping sphincteroplasty is a durable repair for large anal sphincter defects
