Purpose/Background: To evaluate the efficacy and safety of laser ablation with closure for sacrococcygeal pilonidal disease and to identify risk factors for postoperative recurrence in men.

Methods/Interventions: We retrospectively reviewed patients who underwent laser ablation with closure between March 2019 and December 2024. Outcomes included cure, recurrence, and postoperative complications. Logistic regression was used to examine risk factors for recurrence.

Results/Outcomes: We included 267 patients, 218 men (81.6%); 41 patients (15.4%) were ≤18 years. The disease duration was 24 months (IQR 3-71), and BMI was 25.88 kg/m² (IQR 23.44-28.72). Sixty-three patients had a history of smoking and 46 of alcohol use; 65 had prior incision and drainage, and 36 had undergone non-laser surgery (excision with closure or flap). Median follow-up was 24 months (IQR 17-36). The overall cure rate was 100%. Median wound-healing time was 36 days (IQR 32-41), and time to resume normal work and daily activities was 7 days (IQR 6-12). Postoperative analgesics were not required in 92.9% of patients. No wound infections occurred. Overall satisfaction was 97.8% (Table 1). The overall recurrence rate was 5.6% (15/267), all in men. Of the 15 patients with recurrence, 13 underwent repeat laser ablation with closure and 2 underwent incision; all reoperations achieved cure. No recurrences occurred among the 49 women; because stable multivariable estimates were not achievable, risk-factor analyses were restricted to men (n=218). In multivariable analysis, age ≤18 years (OR 4.764, 95% CI 2.424-34.905) and prior pilonidal surgery (OR 5.078, 95% CI 1.431-18.019) were independent predictors of recurrence (both P< 0.05) (Table 2).

Conclusion/Discussion: Laser ablation with closure is minimally invasive, enables rapid recovery, and yields high patient satisfaction. Young men and those with a history of pilonidal surgery have a higher risk of recurrence and warrant closer follow-up.
