Purpose/Background: Hesperidin and diosmin are naturally derived flavonoids commonly used in combination (commercially known as H Formula) for the treatment of hemorrhoidal disease. These compounds exert anti-inflammatory, venotonic, and capillary-protective effects. Multiple randomized controlled trials and meta-analyses have demonstrated their efficacy in reducing hemorrhoidal symptoms such as pain, bleeding, and prolapse severity, particularly in acute Grade I-III internal hemorrhoids. Despite extensive use in Europe, hesperidin/diosmin therapy is underutilized in routine U.S. proctologic practice.

Methods/Interventions: This is a prospective, single-center observational study. A total of 50 adult patients presenting with symptomatic Grade I-III internal hemorrhoids were enrolled. The patients filled out a 5 question survey online pre- and post- use of a hesperidin and diosmin supplement for 30 days. Symptoms evaluated on a Likert scale include bleeding, burning, itching, pain, and prolapse.

Results/Outcomes: 50 patients were surveyed during a period between May 2025 to October 2025. Demographics and comorbidity data was gathered. 92% of patients reported improvement in their symptoms and this corresponded to their level of satisfaction.

Conclusion/Discussion: Hesperidin and diosmin supplements have been widely used in Europe and Asia for the treatment of hemorrhoids and varicose veins and there is sufficient data that these supplements are effective. Despite this, these products are not widely used in the USA. As colorectal surgeons who treat a wide range of proctologic diseases, exposure to another potential tool in our tool belt is important. Based on our survey, patients reported improvement in their symptoms after taking the supplement; therefore hesperidin and diosmin could be useful adjuncts to standard treatments for hemorrhoids. Further studies should assess long-term outcomes and potential side effects of these supplements.
