Purpose/Background: With increasing volume of patients undergoing watch and wait for management of rectal cancer, there are increasing discussions regarding operative volumes, centralization of care, and the impact on trainees. This study evaluates the operative volumes of rectal cancer in a tertiary care, National Accreditation Program for Rectal cancer (NAPRC) designated center.

Methods/Interventions: After institutional review board approval, the NAPRC database was queried for new rectal cancer diagnoses seen at our center from 2020 through 2024. Prevalence of rectal cancer cases as well as operative incidence were reported longitudinally.

Results/Outcomes: Overall, we identified 409 new rectal cancer diagnoses during the time interval. During the timespan, 197 cases required operative intervention. Operative incidence varied from 46% in 2021 to 50% in 2023. Comparing 2020, when our center became a NAPRC center, to 2024, the prevalence of rectal cancer diagnosis being evaluated grew 10.1%, while the operative rates grew 19.4%. However, year to year there is a largely stable volume of rectal cancer new diagnoses and operative cases. (Figure)

Conclusion/Discussion: NAPRC centers aim to centralize and standardize rectal cancer treatment strategies in a changing clinical landscape. This study recognizes the operative volume at a relatively new NAPRC center. The volume has withstood both the COVID 19 pandemic, integration of total neoadjuvant therapy into National Comprehensive Cancer Network (NCCN) guidelines, and the addition of a second NAPRC center within the city limits. Analysis of this data is necessary to ensure operative volumes are adequate for trainees at fellowship programs and should be analyzed routinely given the changing clinical guidelines and practice patterns.
