Purpose/Background: Database studies suggest that the incidence of diverticulitis is increasing among young patients (ages 45 years and younger) with higher severity of disease. However, granular data detailing disease specifics are lacking.

Methods/Interventions: We performed a retrospective database review of patients diagnosed with diverticulitis between 2019-2021 to assess diverticulitis severity among patients with early onset versus standard onset disease. All adult patients evaluated in the emergency department or surgery clinic at our academic-affiliated community hospital system were included. Manual chart review was performed and demographic and clinical data for patients was recorded. Chi-squared and student's t test were used to assess for significant differences between the two cohorts.

Results/Outcomes: A total of 186 patients were included, of which 47 were aged 45 years or younger at the time of diverticulitis onset and therefore considered to have "early onset" disease. Obese BMI and tobacco use patterns were similar between the early onset and standard onset groups (p>0.05). Young patients were predominantly male (p < 0.05), with equivalent rates of complicated diverticulitis to the older cohort. Younger patients were more likely to be diagnosed via ER visit. Young patients also had a shorter average hospital length of stay were less likely to undergo emergency surgery, but rates of fecal diversion (either diverting loop ileostomy or end colostomy) were similar. Results with asterisk indicate statistical significance.

Conclusion/Discussion: Among our dataset, patients with early onset diverticulitis had equivalent rates of complicated disease as compared to standard onset patients. Prospective and longitudinal data are needed to ascertain evolution of diverticular disease over time, to identify risk factors for development of early onset disease, and to predict need for operative intervention in order to reduce surgical morbidity.
