Purpose/Background: This is a case of a 56 year old male who initially presented with a sigmoid colon perforation due to diverticulitis at an outside hospital, which required a laparotomy with resection. His care was complicated by ongoing ischemia of the colon and colostomy. After numerous resections he was left with only the cecum, which was matured as a cecostomy. Pre-operative flexible sigmoidoscopy and stoma colonoscopy demonstrated a stenotic cecostomy. He was scheduled for colostomy reversal with ileorectal anastomosis.

Methods/Interventions: The patient underwent an extensive lysis of adhesions performed both laparoscopically and robotic, lasting longer than 5 hours followed by robotic assisted colostomy reversal, low anterior resection with ileorectal anastomosis. Intra-operative findings re-demonstrated stenotic cecostomy and severe adhesions throughout the abdomen and extensive pelvic fibrosis.

Results/Outcomes: The patient was discharged home within 24 hours of his operation. He was seen in the outpatient setting tolerating a diet, having regular, continent bowel movements, and undergoing regular activity.

Conclusion/Discussion: Here we discuss the importance of implementing the robotic platform in extremely challenging colorectal cases which offer the opportunity to enhance the performance of difficult dissection, as seen in this procedure by providing superior three-dimensional visualization, articulated instrumentation, and improved ergonomics. These features allow for precise dissection in confined pelvic spaces, better nerve preservation, and reduced conversion rates in technically demanding cases, such as in this patient with extensive pelvic fibrosis. This video presentation showcases the capabilities of robotic surgery in this unique case.
