Purpose/Background: We present a case of a robotic transverse colectomy with a handsewn intracorporeal colocolonic anastomosis for an endoscopically unresectable transverse colon polyp.

Methods/Interventions: A 59 year old male was referred to our clinic with a large endoscopically unresectable colonic polyp located in the mid-transverse colon. Previous attempts at endoscopic mucosal resection were incomplete with residual polypoid tissue. He decided to proceed with a surgical resection. We first entered the lesser sac and dissected the gastrocolic ligament off of the transverse colon. The flexures were mobilized to ensure a tension-free anastomosis later in the case. The middle colic artery and vein were isolated and the vessels were skeletonized and divided between clips. The mesocolon was divided and the colon transected ensuring appropriate proximal and distal margins. A handsewn two-layer end-to-end colocolonic anastomosis was then performed The specimen was extracted through a Pfannenstiel incision.

Results/Outcomes: Postoperative pathology showed an adenomatous polyp that was negative for dysplasia or malignancy with negative margins. The patient recovered well and was discharged home on postoperative day 3.

Conclusion/Discussion: This video highlights a minimally invasive approach to a challenging case. A handsewn intracorporeal anastomosis is a skill, which is particularly useful in cases where reach does not allow for a stapled side-to-side anastomosis.
