Purpose/Background: Continent ileostomy (also known as K pouch) is a technique to provide better quality of life in patients otherwise deemed to have conventional ileostomy. Nowadays, few surgeons are familiar with continent ileostomy surgery but it still has a role in carefully selected patients who are either unsuitable for an IPAA, have a failed IPAA that cannot be salvaged, or whose conventional ileostomy is not consistent with an adequate quality of life. We aimed to describe our technique to create K pouch in a patient who had a failed J pouch.

Methods/Interventions: Our patient was a 37 year old female who had a J pouch due to medically refractory ulcerative colitis which got complicated with anastomotic strictures requiring multiple dilations. She had poor sphincter function, therefore was not a candidate for re-do IPAA. K pouch was recommended.

Results/Outcomes: We performed a K pouch successfully on our patients. She had no complications postoperatively.

Conclusion/Discussion: The continent ileostomy retains an important, although small, role in the surgical treatment of patients with inflammatory bowel disease and hereditary colorectal cancer syndromes. Continent ileostomy provides an alternative to the conventional ileostomy for patients who are not candidates for IPAA or who have an existing ileostomy that is adversely affecting their lifestyle.
