Purpose/Background: Pyoderma gangrenosum (PG) is reported in approximately 1-5% of patients with ulcerative colitis (UC). Although the relationship between PG and UC is well established, the outcomes of these patients after ileal pouch-anal anastomosis (IPAA) is not well studied. Given the known relationship between these two diseases, it is important to determine whether PG in UC patients is linked to differences in postoperative outcomes following IPAA.

Methods/Interventions: We included all patients who had primary or re-do IPAA between September 2016 to December 2024 at a tertiary academic medical center. We used 1:4 propensity score matching between groups with and without PG. We compared short-term complications and long-term pouch survival between the two cohorts.

Results/Outcomes: We matched 22 patients with PG (primary IPAA, n=15; re-do IPAA, n=7) with 88 patients without PG. Patient characteristics including age, sex, body mass index, and smoking history were comparable between PG and no-PG groups after matching (Table 1-2). In addition, operative characteristics (i.e. pouch configuration and anastomosis type) and postoperative outcomes including readmissions and septic and non-septic complications were comparable between the two groups (Tables 2 and 3). Long-term pouch survival was similar between the two groups in median follow up of 3.5 years (5 year survival PG: 88%, no PG: 89%, p=0.89). These findings were consistent across both primary and re-do IPAA groups.

Conclusion/Discussion: Pyoderma gangrenosum was not associated with worse outcomes in patients undergoing primary or re-do IPAA for ulcerative colitis. Larger scale studies are needed to identify true associations between PG and outcomes after IPAA.
